If you want to join the Breakfast Optimist Club of Kalamazoo, BOCK:

• Suggestion: Download this PDF file and open using Adobe Acrobat Reader

• Make the needed entries on the membership form,

• If you have a BOCK sponsor enter sponsor’s name and leave the signature field empty.

• If no sponsor, leave both the sponsor name and signature fields empty.

• When done be sure to save the membership form,

• Attach the completed membership form to the Join Form below,

• Enter your first name, last name, and email address on the Join Form below,

• If you want to, include a message,

• To complete the process select the SEND button.
